Parental Consent, Medical Authorization and Release Form

For your safety we require a health form on file for every one who participates in youth events. Health forms must be completed prior to all mission trips, as well as many other outings. See Rob Howard if you have a question as to whether or not you need to complete a health form before participating in a youth sponsored event. If your health insurance or personal information changes, please submit an updated form. You may mail it to the church, bring it to the office, or give it to Rob Howard, Director of Youth Ministries.

Please make sure you submit a copy of BOTH sides of your medical insurance card, along with the health form.
